Contract | Days (5×8 Hours) | Monday–Friday with alternating weekend shifts; hours may vary based on office needs

Key Responsibilities

• Conduct skilled home visits (expected 6–8 per day) for Medicare, Medicare Advantage, and commercial insurance patients
• Perform cardiopulmonary assessments, wound care (basic wound care, ABI compression dressing, wound vac dressing), and post-operative care
• Manage IV therapy including PICC line and Midline care and dressing changes, SQ port access/deaccess, and Pleurx/Aseptdrain care
• Complete OASIS documentation and self-schedule visits based on home care office needs using provided technology (laptop, MiFi, portable printer)
• Administer SQ/IM injections, perform lab draws and venipuncture, foley catheter care and changes, staple/suture removal, and use of glucometer and pro-time machine
• Manage high-risk diagnoses including heart failure, COPD, MI, pneumonia, and stroke

Required Qualifications

• Active Florida RN license or valid Compact RN license required at time of submission
• BCLS certification required
• Minimum 1 year of skilled home health experience required; private duty nursing experience alone will not be accepted
• OASIS training and experience required
• Infusion experience and IV skills required
• Valid driver’s license and personal vehicle required for home visits
• COVID-19 vaccination required or approved exemption on file; flu shot or declination required with mandatory mask wearing if declined
• Candidates must reside more than 100 miles from the facility to be eligible

Preferred Experience / Skills

• Cerner EMR experience preferred
• Experience with general medical diagnoses and high-risk conditions such as heart failure, COPD, MI, pneumonia, and stroke
• Willingness to float within the assigned service area or to a nearby affiliate location
