Key Responsibilities

• Conduct skilled home health visits, with an expected 6–8 patient visits per day
• Perform comprehensive patient assessments including cardiopulmonary/circulatory evaluation, neuro, and orthopedic conditions
• Develop and implement Home Exercise Programs and provide patient and family education
• Utilize and assess adaptive equipment and devices including gait belts, walkers, wheelchairs, sliding boards, and upper extremity prosthetics
• Complete accurate and timely clinical documentation including OASIS assessments and billing
• Recognize and report signs and symptoms of physical, sexual, and psychological abuse; apply safety and injury prevention principles

Required Qualifications

• Active Florida or Compact Physical Therapist license required at time of submission
• BLS certification required
• Minimum 1 year of clinical experience required
• Home health experience required
• Valid driver’s license and personal vehicle required to conduct home visits
• Must be COVID-19 vaccinated or have an approved exemption on file
• Must have current flu shot or declination on file; mandatory mask wearing required at all times while on assignment
• Locals are accepted — candidates residing within 100 miles of the facility may be eligible for this position

Preferred Experience / Skills

• OASIS training preferred
• Cerner EMR experience preferred
• Experience treating a diverse patient population including ortho, neuro, cardiac, COPD, CHF, CVA, Parkinson’s, and post-surgical patients
