Travel RN Case Manager

Schedule / Job Type

Contract | Travel | Days | 8-hour shifts, five days per week | 13-week assignment | Projected start: October 27, 2026 | 9:00 AM – 5:30 PM | Weekend and holiday rotation expected

Key Responsibilities

• Conduct patient assessments and clinical evaluations across assigned hospital units
• Manage discharge planning in collaboration with patients, families, and the multidisciplinary team
• Facilitate and lead multidisciplinary rounds (MDR) discussions in a proactive, autonomous manner
• Coordinate referrals and post-acute care transitions using the CarePort platform
• Document case management activities in Midas and review clinical information in Meditech
• Maintain a patient caseload ratio of 1:25 on general floors and 1:20 in intensive/critical care units

Required Qualifications

• Active RN license required
• Minimum 1 year of experience in hospital and/or case management
• Proficiency with iPad, computer, and cell phone-based clinical tools
• Ability to work every other weekend and holidays in rotation with facility staff

Preferred Experience / Skills

• Experience with Midas, Meditech, and/or InterQual
• Background in acute hospital case management
• Familiarity with the CarePort referral platform
• Strong critical thinking, troubleshooting, and independent facilitation skills
