LPN / LVN (Licensed Practical Nurse)

Schedule / Job Type

Contract | Days | 8-hour and 12-hour shifts available | Every other weekend required | Every other holiday required

Key Responsibilities

• Provide direct nursing care to residents with a variety of diagnoses including CHF, Dementia, Alzheimer’s, Diabetes Mellitus, A-fib, and CVA-related conditions
• Manage and monitor residents requiring ostomy care, catheter care, and wound management
• Administer medications and treatments in accordance with physician orders and facility protocols
• Utilize the PCC electronic medical records system to document resident care accurately and in a timely manner
• Maintain a patient ratio of approximately 1:17 across North and South unit orientations
• Perform IV therapy as required — IV Certification (IV-C) is required for LPNs in this role

Required Qualifications

• Active and unrestricted LPN or LVN license in the state of Nebraska or a compact state
• IV Certification (IV-C) required
• Experience working in a long-term care or skilled nursing facility setting
• Proficiency or ability to quickly learn PCC (PointClickCare) EMR system
• Must live more than 80 miles from the facility to be eligible for standard contract rates

Preferred Experience / Skills

• Prior experience caring for patients with dementia, Alzheimer’s, or behavioral health diagnoses such as depression and anxiety
• Familiarity with managing complex chronic conditions including CHF, A-fib, GERD, and hyperlipidemia
• Experience with catheter and ostomy care in a long-term care environment
