Speech Language Pathologist (SLP)

Santa Clara, CA | Post-Acute Care

Schedule / Job Type

Contract (13 Weeks) | Days | Monday – Friday | Onsite

Key Responsibilities

• Evaluate and treat patients with speech, language, cognitive, and swallowing disorders in a post-acute care setting
• Develop and implement individualized treatment plans based on patient evaluation findings
• Monitor and document patient progress toward established therapy goals
• Collaborate with interdisciplinary care team members to coordinate patient care
• Educate patients and families on therapeutic strategies and home programs

Required Qualifications

• Active California Speech Language Pathologist (SLP) license required
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA required
• Master’s degree in Speech-Language Pathology or Communication Disorders
• Current BLS/CPR certification

Preferred Experience / Skills

• Prior experience in a post-acute, skilled nursing, or rehabilitation setting
• Experience with dysphagia evaluation and treatment
• Familiarity with electronic medical records (EMR) documentation
