Contract | Night Shift | 12-Hour Shifts
Key Responsibilities
- Provide direct patient care to long term care residents during night shift hours
- Administer medications and treatments as prescribed, following facility protocols
- Monitor and document patient conditions, reporting changes to the supervising RN or physician
- Collaborate with interdisciplinary care team to support individualized resident care plans
- Ensure a safe and comfortable environment for residents throughout the shift
Required Qualifications
- Active LPN license in the state of New York
- Experience working in a long term care or skilled nursing environment
- Ability to work 12-hour night shifts
- Current CPR/BLS certification
Preferred Experience / Skills
- Prior travel or contract nursing experience
- Familiarity with electronic health records (EHR) systems
- Strong communication and critical thinking skills in a long term care setting
