Key Responsibilities

• Provide physical therapy services to patients across assigned care settings
• Evaluate patients and develop individualized treatment plans to support functional recovery and rehabilitation goals
• Deliver hands-on therapeutic interventions and monitor patient progress throughout the course of care
• Collaborate with interdisciplinary care teams to coordinate patient treatment and discharge planning
• Maintain valid state-issued photo identification and wear a photo badge at all times while on facility grounds

Required Qualifications

• Active Physical Therapist (PT) license in the state of Washington
• Graduate of an accredited Physical Therapy program
• Valid state or federal issued photo identification required

Preferred Experience / Skills

• Prior experience in a skilled nursing or long-term care setting
• Ability to adapt to a split-site work environment
• Strong communication and interdisciplinary team collaboration skills
