Key Responsibilities

• Evaluate and treat residents in a skilled nursing facility setting to restore and maintain functional mobility and independence
• Develop and implement individualized physical therapy treatment plans based on patient assessments and goals
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
• Educate patients, families, and caregivers on therapeutic exercises and home programs

Required Qualifications

• Active Physical Therapist (PT) license in the state of Washington or eligibility to obtain
• Graduate of an accredited Physical Therapy program
• Ability to work full-time, 40 hours per week, Monday through Friday

Preferred Experience / Skills

• Prior experience working in a skilled nursing facility or long-term care environment
• Familiarity with geriatric patient populations and related conditions
• Experience with therapy documentation and electronic health record systems
