Contract (13 Weeks) | Days | Wednesday & Friday | Onsite

Key Responsibilities

• Provide direct patient care in a long-term care / skilled nursing facility setting
• Administer medications and treatments as directed by the care plan and supervising clinicians
• Monitor and document patient conditions, reporting changes to appropriate clinical staff
• Collaborate with interdisciplinary team members to support resident health and safety
• Maintain valid state LPN/LVN licensure and adhere to all facility compliance requirements at all times

Required Qualifications

• Active and unrestricted LPN or LVN license in the state of New Mexico
• Current CPR/BLS certification
• Must present a valid state or federal issued photo ID and wear a photo badge at all times while on assignment
• Must have completed the original or updated COVID-19 vaccine series, or be able to present a documented religious and/or medical exemption from your employer

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility environment
• Familiarity with electronic health records (EHR) and clinical documentation systems
• Strong communication and teamwork skills within a multidisciplinary care setting
