Care Manager (Home Health) Registered Nurse

Key Responsibilities
- Complete initial and ongoing comprehensive assessments of patient needs at appropriate time points
- Implement and update individualized plans of care in collaboration with the patient and interdisciplinary care team
- Provide direct and indirect skilled nursing care to patients in their home environment
- Counsel and educate patients and family members on healthcare needs, nursing care, and patient rights
- Accurately document all patient care, status, responses to treatment, and therapeutic interventions in accordance with established policies, including within EPIC
- Coordinate patient care across disciplines and initiate appropriate preventative and rehabilitative nursing procedures
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice (California)
- Current Basic Life Support (BLS) certification from an approved vendor
- Minimum of two years of recent nursing experience
- Valid Driver’s LicenseCurrent automobile insurance and access to a reliable personal vehicle
- EPIC electronic health record experience required
Preferred Experience / Skills
- Bachelor’s Degree in Nursing (BSN)
- Prior acute care facility or home health experience
