Key Responsibilities

• Complete evaluations to ensure quality standards and address assessment areas requiring attention
• Establish individualized treatment plans using appropriate clinical techniques, tools, and treatment protocols
• Provide therapy interventions in a home health setting, developing individualized care and discharge plans and communicating them to appropriate parties
• Complete comprehensive and accurate clinical documentation within established deadlines
• Serve as co-leader in an interdisciplinary approach to patient care
• Supervise assigned therapy assistants as applicable

Required Qualifications

• Active Physical Therapy license in the state of California required
• Doctor of Physical Therapy (DPT) degree required if licensure was obtained on or after January 2016, or foreign equivalent
• Current CPR/BLS certification from an approved vendor required
• Valid Driver’s License; must be at least 21 years of age
• Minimum of two years of recent Physical Therapy experience
• Epic EMR experience required

Preferred Experience / Skills

• Prior home health experience strongly preferred
• Familiarity with interdisciplinary care team collaboration
• Experience developing and communicating discharge planning across care settings
