Contract | Night Shift | 12-Hour Shifts

Key Responsibilities

• Provide direct patient care to long term care residents during night shift hours
• Administer medications and treatments as prescribed, following facility protocols
• Monitor and document patient conditions, reporting changes to the supervising RN or physician
• Collaborate with interdisciplinary care team to support individualized resident care plans
• Ensure a safe and comfortable environment for residents throughout the shift

Required Qualifications

• Active LPN license in the state of New York
• Experience working in a long term care or skilled nursing environment
• Ability to work 12-hour night shifts
• Current CPR/BLS certification

Preferred Experience / Skills

• Prior travel or contract nursing experience
• Familiarity with electronic health records (EHR) systems
• Strong communication and critical thinking skills in a long term care setting
