Key Responsibilities

• Provide direct patient care to residents in a long-term care/skilled nursing facility setting
• Assist residents with activities of daily living including bathing, dressing, grooming, and mobility
• Monitor and report changes in resident condition to nursing staff
• Maintain accurate documentation in accordance with facility and regulatory standards
• Wear a facility-issued photo badge at all times while on assignment

Required Qualifications

• Active LNA licensure in the state of New Hampshire
• Valid state or federal issued photo identification required for facility check-in
• Proof of completed COVID-19 vaccination series, or a documented religious and/or medical exemption from employer; additional doses required where mandated by law
• Must maintain reliable attendance; no more than 2 call-outs and no no-call/no-shows for the duration of the contract

Preferred Experience / Skills

• Previous experience in a long-term care or skilled nursing facility environment
• Familiarity with mid-shift workflows and staffing expectations in a SNF setting
