Key Responsibilities
- Perform and document comprehensive patient assessments in accordance with RN scope of practice, considering culture, age, developmental level, and risk factors
- Develop and implement individualized plans of care in coordination with patients, families, and the inter-professional team
- Recognize, respond to, and communicate changes in patient condition; conduct ongoing assessments and document responses to procedures and therapies
- Facilitate admission, transfer, and discharge activities including timely execution of discharge orders, discharge teaching, and coordination with Case Management
- Delegate tasks appropriately and monitor accuracy and completeness of patient care provided by other team members
- Actively participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing
Required Qualifications
- Active, unrestricted Registered Nurse (RN) license in the state of Kentucky
- Basic Life Support (BLS) certification required
- Associate Degree in Nursing (ADN) or Associate of Science in Nursing (ASN)
- Ability to communicate and collaborate effectively with all members of the care team
- Proficiency with or ability to be trained on computer systems and clinical applications
Preferred Experience / Skills
- Previous clinical nursing experience in a hospital or acute care setting
- Bachelor of Science in Nursing (BSN)
- Experience in Medical Surgical or Telemetry nursing
