Key Responsibilities
- Provide and document nursing care for patients in the home setting, promoting, maintaining, or restoring health and supporting rehabilitation
- Collaborate with healthcare providers, professionals, and community representatives to coordinate individualized care at home
- Evaluate patient and family responses to nursing interventions and adjust care plans accordingly
- Function independently under general guidance, exercising independent judgment and critical thinking in varied clinical situations
- Utilize time management, communication, planning, delegating, and problem-solving skills in daily care delivery
- Accept on-call responsibilities and maintain flexibility regarding work hours and assignments, including required holiday coverage blocks
Required Qualifications
- Active Massachusetts LPN license (must be provided at time of submission)
- Active BLS certification from the American Heart Association or American Red Cross
- Minimum one year of acute nursing care experience; hospital-based experience preferred
- Home health experience required
- Valid driver’s license with active auto insurance; personal vehicle required for home visits
- Must live at least 50 miles from the facility to be eligible
Preferred Experience / Skills
- Certified Home Health experience, including proficiency with OASIS documentation
- Infusion therapy experience
- Experience with self-scheduling of patient caseload
- Working knowledge of home care reimbursement and regulatory guidelines
