Contract | Days | Flexible scheduling available, open to 10-hour shifts or 4-day work weeks

Key Responsibilities

• Provide physical therapy services to home health patients in a community-based setting
• Manage an expected caseload of approximately 4–5 patients per day, based on a weighted visit model
• Conduct evaluations, develop individualized treatment plans, and deliver skilled therapeutic interventions
• Travel to patient homes within the designated service area using a personal vehicle
• Document patient progress and maintain accurate clinical records in accordance with facility standards

Required Qualifications

• Active California Physical Therapist (PT) License
• Minimum of 1 year of experience as a Home Health Physical Therapist
• Valid driver’s license and personal auto insurance required for home visit travel
• Must live at least 50 miles from the facility to be eligible as a travel candidate; local candidates are accepted at the local rate

Preferred Experience / Skills

• Basic Life Support (BLS) certification preferred (AHA or ARC accepted)
• Experience managing a home health caseload with productivity expectations
• Familiarity with e-learning and electronic documentation platforms
