Registered Nurse – Case Management

Contract | Days | Monday–Friday, 8:00 AM–4:30 PM | No weekends | No on-call

Key Responsibilities

• Perform acute inpatient case management functions including utilization review, level of care determinations, and observation status assessment
• Apply knowledge of healthcare reimbursement systems (HMO, PPO, PPS, CMS, and value-based models) to support appropriate patient care transitions
• Collaborate with physicians, nurses, payers, patients, and families to coordinate care and facilitate discharge planning
• Ensure compliance with applicable laws and regulations including CMS, Title 22, CDPH, and TJC standards
• Communicate effectively with internal and external stakeholders to promote quality patient care and a positive patient experience
• Manage a high-volume caseload with rapidly changing priorities while exercising sound, independent clinical judgment

Required Qualifications

• Active California RN license (must be verifiable via Nursys, uploaded within the last 2 weeks)
• Minimum 2 years of registered nursing experience
• Acute inpatient Case Management experience in California is required — outpatient, home health, or health plan case management experience will not be considered equivalent
• Kaiser-based acute case management experience does not meet the experience requirement
• All certifications must be current and issued by AHA or ARC
• Candidates must live within 50 miles of the facility to be considered at the local rate; locals are accepted

Preferred Experience / Skills

• Familiarity with coding and DRG assignment processes
• Knowledge of post-acute levels of care and associated regulatory compliance requirements
• Experience working within a managed care or integrated delivery system environment
• Strong negotiation skills and intermediate computer/technology proficiency
