Key Responsibilities
- Provide direct patient care to long-term care and skilled nursing facility residents in accordance with facility policies and applicable state regulations
- Administer medications and treatments as prescribed, documenting all care accurately and in a timely manner
- Monitor and report changes in patient condition to supervising nursing staff or physicians
- Collaborate with interdisciplinary care teams to support individualized resident care plans
- Maintain and present valid state or federal issued photo identification at all times while on assignment
Required Qualifications
- Active LPN license in the state of Vermont
- Completion of original or updated COVID-19 vaccination series, or a documented religious and/or medical exemption from employer
- Must comply with any additional vaccine or booster requirements where mandated by law
- Local candidates only
Preferred Experience / Skills
- Prior experience in a long-term care or skilled nursing facility setting
- Familiarity with electronic health records and clinical documentation systems
