Key Responsibilities

• Evaluate patients of all ages, perform physical therapy diagnoses, and design individualized plans of care integrating goals, treatment, outcomes, and discharge planning
• Perform examinations using reliable and valid methods relevant to the chief complaint, screening results, and patient history
• Adjust care based on patient response and available evidence, including assessment of orthopedic and neurologic conditions with objective testing for range of motion, strength, and functional tasks
• Complete daily documentation and charging in the therapy EMR in accordance with clinical process policies
• Apply therapeutic tape and modalities such as ultrasound, interferential, and iontophoresis as appropriate
• Educate patients, families, caregivers, and other healthcare providers using relevant teaching methods, and communicate patient progress and treatment recommendations to referring providers

Required Qualifications

• Bachelor’s degree or higher in Physical Therapy
• Active Illinois Physical Therapist (PT) License
• BLS certification through the American Heart Association (AHA)
• At least one (1) year of relevant physical therapy experience
