Key Responsibilities

• Perform comprehensive patient evaluations including history intake, systems review, and administration of tests and measures to assess functional deficits and mobility limitations
• Establish individualized plans of care and select treatment techniques that address identified impairments in accordance with physician orders
• Conduct ongoing reassessment of patients and adjust treatment approaches based on response to care
• Initiate and manage discharge planning beginning at the first visit and continuing throughout the course of treatment
• Supervise and delegate to ancillary staff and students in accordance with state laws and department policy
• Complete accurate, legible documentation within established timeframes and in compliance with accreditation and insurance requirements

Required Qualifications

• Minimum of 2 years of Physical Therapist experience in a Long Term Care setting
• Active Texas PT license or compact state license required
• Must function within the scope of practice as defined by the Texas state practice act

Preferred Experience / Skills

• Experience communicating effectively with patients, families, caregivers, and interdisciplinary healthcare teams
• Demonstrated participation in quality improvement activities and professional development initiatives
