Key Responsibilities

• Provide direct physical therapy services to residents in a skilled nursing facility setting
• Evaluate patients and develop individualized treatment plans to support functional recovery and rehabilitation goals
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Maintain and present a valid state or federal issued photo ID at all times while on site

Required Qualifications

• Active Physical Therapist (PT) license in the state of New Hampshire
• Current CPR/BLS certification as required
• Ability to fulfill a 14-week contract assignment
• Must provide a valid state or federal issued photo ID upon arrival and wear a visible photo badge at all times while at the facility

Preferred Experience / Skills

• Prior experience working in a skilled nursing facility or long-term care environment
• Familiarity with geriatric patient populations and rehabilitation protocols
