PCT – Extended Care (Certified Nursing Assistant)

Key Responsibilities
- Provide all activities of daily living (ADL) care to patients, including oral care, grooming, personal hygiene, dressing, bathing, range of motion, ambulation, and dining assistance
- Deliver care according to each patient’s individualized care plan, respecting dignity, privacy, and promoting independence whenever possible
- Promptly answer call lights and communicate changes in patient condition to the supervising nurse in a timely manner
- Complete required charting and documentation in accordance with facility policies and procedures
- Collaborate with the Interdisciplinary Team to communicate resident preferences and needs to support individualized care planning
- Maintain cleanliness of wheelchairs and adaptive equipment; report unsafe or non-working equipment or fixtures to maintenance; use all required safety equipment appropriately, including gait belts, goggles, lifts, and transfer boards
Required Qualifications
- Active CNA certification in the state of practice (Montana)
- BLS certification through the American Heart Association
- Prior experience as a CNA in an extended care or long-term care setting
- Ability to read and write at a level sufficient to fulfill documentation requirements
- Ability to meet physical demands of the role, including frequent lifting and carrying up to 50 lbs. and occasional lifting of objects over 100 lbs.
Preferred Experience / Skills
- At least one (1) year of experience in a healthcare setting
- High School Diploma or equivalent
