LPN (Licensed Practical Nurse)

St. Albans, VT | Skilled Nursing Facility

Schedule / Job Type

Contract (13 Weeks) | Days | Every Weekend | 16-Hour Shifts (7:00 AM – 11:00 PM) | Onsite

Key Responsibilities

• Provide direct patient care in a long-term care / skilled nursing facility setting
• Administer medications, treatments, and nursing interventions per physician orders
• Monitor and document patient conditions, reporting changes to supervising staff as appropriate
• Collaborate with interdisciplinary care teams to support resident health and wellness goals
• Maintain valid state-issued photo identification and wear a photo badge at all times while on assignment

Required Qualifications

• Active LPN license in the state of Vermont (or compact equivalent accepted)
• Must be a local candidate — local applicants only are being considered for this assignment
• Must have completed the original or updated COVID-19 vaccine series, or be able to present a documented religious and/or medical exemption from your employer
• Must provide a valid state or federal issued photo identification upon arrival at the facility

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility environment
• Familiarity with weekend shift scheduling and extended shift formats
• Strong documentation and communication skills within a multidisciplinary team
