Contract (11 Weeks) | Mid Shift | Onsite

Key Responsibilities

• Provide direct patient care in a long-term care / skilled nursing facility setting
• Administer medications and treatments in accordance with physician orders and facility protocols
• Monitor and document patient conditions, reporting changes to supervising nursing staff
• Collaborate with interdisciplinary care teams to support resident health and wellness goals
• Maintain compliance with facility policies, including wearing a photo ID badge at all times
• Fulfill a weekend commitment every weekend throughout the duration of the assignment

Required Qualifications

• Active LPN license in the state of Vermont
• Must be a local candidate — local applicants only
• Valid state or federal issued photo identification required for facility check-in
• Must have reliable personal transportation, as public transportation in the area is limited
• COVID-19 vaccination required, or a documented religious and/or medical exemption from employer
• All requested time off must be submitted and approved prior to contract finalization

Preferred Experience / Skills

• Previous experience in a long-term care or skilled nursing facility environment
• Familiarity with electronic health records and clinical documentation systems
• Strong communication and teamwork skills within a multidisciplinary care setting
