Key Responsibilities

• Provide direct patient care to residents in a long-term care / skilled nursing facility setting
• Administer medications and treatments in accordance with physician orders and facility protocols
• Monitor and document patient conditions, reporting changes to the supervising nurse or physician as appropriate
• Collaborate with interdisciplinary care teams to support individualized resident care plans
• Maintain valid state LPN/LVN licensure and adhere to all facility compliance requirements, including wearing a photo ID badge at all times

Required Qualifications

• Active LPN or LVN license valid in the state of New Hampshire
• Must present a valid state or federally issued photo identification upon arrival at the facility
• Must have completed the original or updated COVID-19 vaccine series, or be able to present a documented religious and/or medical exemption from employer; additional doses required where mandated by law
• Local candidates only

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility environment
• Familiarity with electronic health records and clinical documentation systems
• Strong communication and teamwork skills in a fast-paced care setting
