Key Responsibilities
- Prescribe, coordinate, and deliver patient care through the nursing process in accordance with professional nursing standards
- Conduct comprehensive initial and ongoing assessments addressing physical, psychosocial, environmental, and community resource needs across the lifespan
- Initiate, evaluate, and modify Plans of Care as needed; establish discharge plans in collaboration with patients, physicians, other disciplines, and caregivers
- Complete OASIS documentation and all clinical documentation in accordance with Federal, State, and agency regulatory guidelines
- Assess and implement educational strategies for patients, families, and caregivers; perform home visits as well as some telehealth/virtual visits
- Supervise CNAs and coordinate care with other healthcare team members while maintaining patient privacy and confidentiality standards
Required Qualifications
- Home Health nursing experience required
- Active Illinois RN license (must be active at time of submission)
- Active BLS certification or willingness to obtain prior to start
- Valid driver’s license and current auto insurance required
- COVID-19 and Influenza vaccinations required — declinations will not be approved
- Local candidates preferred; candidates located outside the service territory may not be considered
Preferred Experience / Skills
- OASIS and start-of-care experience strongly preferred
- Strong Med/Surg background with some Home Health experience may be considered
- Familiarity with chronic disease management, wound/ostomy care, or infusion management
