Contract | Days, Monday–Friday | 32–40 hours per week

Key Responsibilities

• Provide skilled nursing care to patients within their home environment across the designated service area
• Perform OASIS assessments and develop individualized care plans for in-home patients
• Monitor patient progress and adjust care plans as clinically appropriate
• Participate in the on-call rotation, including nights, weekends, and holidays (approximately one in four)

Required Qualifications

• Active Registered Nurse (RN) license
• Minimum of 2 years of experience performing OASIS assessments and care planning for in-home patients
• Ability to participate in an on-call rotation covering nights, weekends, and holidays

Preferred Experience / Skills

• Prior experience working within the Epic electronic health record system
• Familiarity with home health operations and county-based service delivery models
