Registered Nurse (RN) – Home Health

Contract | Days | Full-Time | 5 x 8-hour shifts per week | Onsite / Field-Based

Key Responsibilities

• Provide home health nursing services to adult patients across a multi-county coverage area including Marion, Washington, Taylor, LaRue, Hardin, Meade, and Breckenridge Counties
• Perform Start of Care (SOC) assessments, recertifications, and daily patient visits, targeting approximately 6.5 points per day
• Case manage a caseload of approximately 30 adult patients
• Document clinical visits and patient records using Home Care Home Base (HCHB) charting software
• Participate in weekend rotation (hours 8am–5pm) and holiday rotation as scheduled
• Utilize agency-provided phone and tablet for field documentation and communication

Required Qualifications

• Active RN license in the state of Kentucky
• Home Care Home Base (HCHB) charting experience required — no exceptions
• Home health nursing background required — no exceptions
• Minimum 1 year of experience in a Med/Surg hospital setting
• Ability to manage an adult-only patient population in a field-based environment

Preferred Experience / Skills

• Experience performing Start of Care assessments and recertifications in a home health setting
• Familiarity with multi-county travel and independent field-based nursing practice
• Strong time management skills to meet daily point productivity expectations
