Schedule / Job Type

Contract | Days | Weekend rotations approximately once every 6 weeks

Key Responsibilities

• Assess patient conditions and develop and implement individualized care plans
• Provide ongoing education and support to patients and their families
• Perform admission processes in accordance with organizational policies and standards of practice
• Carry out case management responsibilities, including thorough and timely electronic documentation
• Communicate effectively across interdisciplinary teams to coordinate patient care
• Travel to assigned patient cases within the service area

Required Qualifications

• Active Physical Therapist license
• Minimum 1 year of experience within a Certified Home Health Agency (experience in hospice, private care, or case management outside of a certified home health agency does not meet this requirement)
• Minimum 1 year of case management experience
• Ability to travel to cases as assigned

Preferred Experience / Skills

• Familiarity with electronic health record documentation systems
• Strong interdisciplinary communication and collaboration skills
• Experience delivering patient care in alignment with community health practice standards
