Key Responsibilities

• Provide, teach, supervise, and document nursing care for patients in the home setting
• Intervene to promote, maintain, or restore health; prevent illness; and support rehabilitation
• Collaborate with healthcare providers, professionals, and community representatives to coordinate home-based care
• Evaluate individual and family responses to nursing interventions and adjust care plans accordingly
• Function independently under general guidance, applying time management, communication, planning, delegating, and problem-solving skills
• Maintain emotional stability in emergency settings and exercise independent clinical judgment; accept on-call responsibilities as needed

Required Qualifications

• Active Massachusetts RN License (must be provided at time of submission)
• Active American Heart Association (AHA) BLS Certification
• Certified Home Health certification with demonstrated proficiency in OASIS (required)
• Minimum one year of acute nursing care experience; hospital-based experience preferred
• Experience in home health (required)
• Valid driver’s license and personal auto insurance; personal vehicle required for this role
• Must reside at least 50 miles from the facility to be eligible

Preferred Experience / Skills

• Infusion Therapy Certification
• Experience with self-scheduling of caseload
• Working knowledge of home care reimbursement and regulatory guidelines
• Bachelor of Science in Nursing (BSN) highly preferred
