LPN (Licensed Practical Nurse)

Schedule / Job Type

Contract (10 Weeks) | Mid Shift | Onsite

Key Responsibilities

• Provide direct patient care in a long-term care / skilled nursing facility setting
• Administer medications and treatments as directed by the care plan
• Monitor and document patient conditions, reporting changes to the supervising nurse or physician
• Collaborate with interdisciplinary care teams to support resident health and well-being
• Rotate weekend shifts as scheduled — every other weekend required

Required Qualifications

• Active LPN license in the state of Vermont
• Must be a local candidate — locals only
• Must have reliable personal transportation
• Must present a valid state or federal issued photo ID at the facility and wear a photo badge at all times
• Must have completed the original or updated COVID-19 vaccine series, or present a documented religious and/or medical exemption from employer
• Additional COVID-19 doses must be obtained where required by law

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility environment
• Familiarity with electronic health records and care documentation systems
