Key Responsibilities

• Evaluate and treat patients within a veterans care and skilled nursing facility setting
• Develop and implement individualized physical therapy treatment plans
• Monitor and document patient progress toward established goals
• Collaborate with interdisciplinary care teams to coordinate patient care
• Maintain accurate and timely clinical documentation in accordance with facility standards

Required Qualifications

• Active Physical Therapist license in the state of Oklahoma (or eligibility to obtain)
• Graduate of an accredited Physical Therapy program
• Ability to work full-time, Monday through Friday with no weekend rotation

Preferred Experience / Skills

• Prior experience in a skilled nursing facility (SNF) setting
• Experience working with veterans or in a VA-affiliated care environment
• Familiarity with geriatric or long-term care patient populations
