Key Responsibilities
- Evaluate and treat residents in skilled nursing and post-acute settings
- Develop and implement individualized treatment plans to help residents achieve their highest functional potential
- Collaborate with an interdisciplinary team to ensure seamless, coordinated care
- Document accurately and timely in accordance with facility and regulatory standards
- Participate in care conferences and contribute to ongoing quality improvement efforts
Required Qualifications
- Active California Physical Therapist (PT) license required
- Strong clinical and communication skills
- Dependable, flexible, and team-oriented
- Commitment to high-quality, compassionate patient care
Preferred Experience / Skills
- Prior experience in skilled nursing or post-acute settings
- Familiarity with interdisciplinary care team collaboration and care conference participation
