Key Responsibilities

• Provide physical therapy evaluation and treatment to residents living with Alzheimer’s and other forms of dementia in a specialized memory support setting
• Develop individualized, person-centered plans of care that maximize function and engagement through environmental accommodations and task modifications
• Incorporate meaningful activities such as art therapy, music therapy, and gardening into daily treatment plans
• Administer and interpret standardized assessments including the BCAT, GDS, and ACL
• Partner with care partners and families as integral members of the care team to sustain quality of life post discharge from therapy

Required Qualifications

• Active Physical Therapist (PT) license in the state of Maine
• Experience working in a skilled nursing, long-term care, or memory care environment
• Ability to work a Monday–Friday schedule with weekend rotation
• Expected 40 hours per week

Preferred Experience / Skills

• Experience working with individuals diagnosed with Alzheimer’s disease or other forms of dementia
• Familiarity with evidenced-based practice frameworks supported by researchers such as Allen, Reisberg, and Mansbach
• Experience with person-centered care models and individualized programming
