Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility serving both short-term and long-term care populations
• Develop and implement individualized physical therapy treatment plans based on patient needs and goals
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Collaborate with interdisciplinary care teams to ensure comprehensive patient care
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

• Active Physical Therapist (PT) license in the state of North Carolina
• Graduate of an accredited Physical Therapy program
• Ability to work effectively with both short-term rehabilitation and long-term care patients

Preferred Experience / Skills

• Prior experience in a skilled nursing facility or long-term care setting
• Experience treating a diverse geriatric patient population
• Strong interpersonal and communication skills for team-based care environments
