Full-Time | Permanent | Onsite | Days | Monday–Friday, No Weekend Rotation

Key Responsibilities

• Evaluate and treat residents with speech, language, cognitive, and swallowing disorders in a skilled nursing facility setting
• Develop and implement individualized treatment plans for post-hospital, short-term rehabilitation, long-term, and respite care residents
• Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to support resident goals and overall care plans
• Provide education and guidance to residents, families, and staff regarding communication and swallowing strategies

Required Qualifications

• Active Speech Language Pathologist (SLP) license in the state of Tennessee
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA preferred or eligibility required
• Experience working in a skilled nursing or long-term care environment
• Strong documentation and communication skills

Preferred Experience / Skills

• Experience with dysphagia evaluation and treatment in a SNF or post-acute setting
• Familiarity with MDS documentation and Medicare/Medicaid compliance in skilled nursing facilities
• Experience working with geriatric and long-term care populations
