Permanent, Direct Hire | Days | Monday through Friday with weekend rotation as needed | Expected 30 hours/week | Onsite

Key Responsibilities

• Provide physical therapy services across a multi-level senior living campus including long-term care, assisted living, and independent living settings
• Evaluate and treat residents from acute episodes through recovery, supporting their return to higher levels of independence within the community
• Continue therapeutic services as residents transition back to their apartments or independent living homes
• Collaborate with a full-time activities department to coordinate rehabilitation services with off-site trips and community programming
• Float to neighboring sites as needed to support continuity of care

Required Qualifications

• Active Physical Therapist (PT) license in the state of New Hampshire or eligibility to obtain
• Ability to work a Monday through Friday schedule with participation in a weekend rotation
• Comfort working across multiple care settings including long-term care, assisted living, memory care, and independent living
• Willingness to float to neighboring sites as needed

Preferred Experience / Skills

• Prior experience in a skilled nursing, assisted living, or senior living rehabilitation setting
• Experience working with geriatric populations across varying levels of care
• Familiarity with memory care or dementia-specific rehabilitation approaches
