Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) – Home Health

Key Responsibilities

• Provide and document nursing care for patients in the home setting, promoting, maintaining, or restoring health and supporting rehabilitation
• Collaborate with healthcare providers, professionals, and community representatives to coordinate individualized care at home
• Evaluate patient and family responses to nursing interventions and adjust care plans accordingly
• Function independently under general guidance, exercising independent judgment and critical thinking in varied clinical situations
• Utilize time management, communication, planning, delegating, and problem-solving skills in daily care delivery
• Accept on-call responsibilities and maintain flexibility regarding work hours and assignments, including required holiday coverage blocks

Required Qualifications

• Active Massachusetts LPN license (must be provided at time of submission)
• Active BLS certification from the American Heart Association or American Red Cross
• Minimum one year of acute nursing care experience; hospital-based experience preferred
• Home health experience required
• Valid driver’s license with active auto insurance; personal vehicle required for home visits
• Must live at least 50 miles from the facility to be eligible

Preferred Experience / Skills

• Certified Home Health experience, including proficiency with OASIS documentation
• Infusion therapy experience
• Experience with self-scheduling of patient caseload
• Working knowledge of home care reimbursement and regulatory guidelines
