Key Responsibilities

• Provide direct nursing care to residents in a long-term care / skilled nursing facility setting
• Utilize PointClickCare (PCC) electronic health records system for documentation and care coordination
• Administer medications and treatments in accordance with physician orders and facility protocols
• Monitor and report changes in resident condition to appropriate clinical staff
• Maintain compliance with facility policies, state regulations, and infection control standards
• Wear a photo ID badge at all times while on assignment; a state or federal issued photo identification is required upon entry to the facility

Required Qualifications

• Active and unrestricted LPN or LVN license in the state of Vermont
• Proficiency with PointClickCare (PCC) electronic health records system
• Prior nursing home or long-term care experience required
• COVID-19 vaccination required, or a documented religious and/or medical exemption from employer; additional doses may be required where mandated by law
• Local candidates only

Preferred Experience / Skills

• Experience working in a skilled nursing facility (SNF) environment
• Familiarity with long-term care regulatory requirements and documentation standards
