Registered Nurse (RN) – Case Manager

Schedule / Job Type

Contract | Days | Five 8-hour shifts per week (10/12-hour shifts are not available for this position)

Key Responsibilities

• Manage the healthcare needs, facilitation, and achievement of quality and cost outcomes for all patients across the continuum of care
• Utilize a multidisciplinary team approach to negotiate, procure, coordinate, and monitor services and resources needed by patients
• Review patients at admission and concurrently throughout the hospital stay to assess appropriate level of care and utilization of resources
• Perform transition care planning activities to secure appropriate post-hospital care arrangements and monitor patients throughout their hospitalization
• Collaborate and negotiate with providers, patients, families, insurance organizations, and government agencies to support optimal patient outcomes
• Participate in performance improvement initiatives and prepare comprehensive summary reports for files, management, legal, and/or regulatory agencies as needed

Required Qualifications

• Active Massachusetts RN License required
• Active BLS certification through American Heart Association or American Red Cross required
• 3+ years of clinical experience in acute care settings such as internal medicine, surgery, orthopedics, mother/child, cardiology, or oncology (experience in Rehab, Home Health, or Long-Term Care may be considered)
• Ability to perform comprehensive admission and discharge risk assessments related to quality, financial, payer benefit allowances and limitations, risk management, and patient satisfaction
• Knowledge of Medicare, Medicaid, third-party commercial payers, and reimbursement requirements
• Knowledge of local community resources and proficiency with computer applications, including the ability to manage multiple open applications simultaneously
• Traveler must have their own vehicle for this assignment
• Must reside at least 50 miles from the facility to be eligible

Preferred Experience / Skills

• CCMA/ACM – Certified Case Manager or Accredited Case Manager certification preferred
• RN-BC – Board Certification in Case Management preferred
• Meditech Expanse experience highly preferred
• Knowledge of regulatory and accreditation organizations such as Joint Commission, DPH, and Healthgrades preferred
