Key Responsibilities
- Provide direct patient care in a long-term care / skilled nursing facility setting
- Administer medications and treatments as directed by the care plan
- Monitor and document patient conditions, reporting changes to the supervising nurse or physician
- Collaborate with interdisciplinary care teams to support resident health and well-being
- Rotate weekend shifts as scheduled — every other weekend required
Required Qualifications
- Active LPN license in the state of Vermont
- Must be a local candidate — locals only
- Must have reliable personal transportation
- Must present a valid state or federal issued photo ID at the facility and wear a photo badge at all times
- Must have completed the original or updated COVID-19 vaccine series, or present a documented religious and/or medical exemption from employer
- Additional COVID-19 doses must be obtained where required by law
Preferred Experience / Skills
- Prior experience in a long-term care or skilled nursing facility environment
- Familiarity with electronic health records and care documentation systems
