Key Responsibilities
- Provide occupational therapy evaluations and treatments to patients in a skilled nursing facility setting
- Develop and implement individualized treatment plans to support patient recovery and functional independence
- Document patient progress and maintain accurate clinical records using NetHealth (Rehab Optima)
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
Required Qualifications
- Active California Occupational Therapist license required
- Minimum of one year of skilled nursing facility (SNF) experience
- Proficiency with NetHealth (Rehab Optima) documentation system
Preferred Experience / Skills
- Experience working in a fast-paced SNF or post-acute rehabilitation environment
- Strong clinical documentation and time management skills
