Key Responsibilities

• Provide home visits to adult patients across an assigned territory, with an average daily caseload of 5–8 patients and a weekly productivity expectation of 30 points.
• Perform case types including Start of Care, Wound Care, follow-up visits, and other adult home health services.
• Complete all clinical documentation within 24 hours of each patient appointment; proficiency in Homecare Homebase (HCHB) is required.
• RNs must be proficient in OASIS and have used it within the past six months, or complete a retraining course during compliance.
• Attend periodic office meetings at the assigned care center in addition to daily patient home visits.
• Travel up to 75 miles from the home clinic to reach patients; accept all scheduled patients regardless of distance from previous appointment.

Required Qualifications

• Active, unrestricted RN license — no pending licenses accepted.
• Minimum of one full year of recent adult home health experience.
• First-time travelers are not eligible; recent specialty experience is required.
• Current CPR and BLS certifications required.
• Proficiency in Homecare Homebase (HCHB) with use within the past six months (candidates with prior HCHB experience outside this window may be considered with retraining during compliance).
• Reliable personal vehicle, valid driver’s license, and agency-required auto liability insurance required; mileage reimbursed at the IRS standard rate for travel between patient homes.
• Local candidates are welcome and accepted at the same rate.

Preferred Experience / Skills

• Experience with OASIS documentation in a home health setting within the past six months.
• Familiarity with a broad range of adult home health case types including wound care and start-of-care visits.
• Comfort with extended driving distances and flexible territory coverage across multiple communities.
