Case Manager – Registered Nurse (Nursing)

Location

Mattoon, IL | Acute Care

Schedule / Job Type

Contract | Full-Time | Days | Monday–Friday, 8:00 AM–4:30 PM | No weekends, major holidays, or call required

Key Responsibilities

• Provide case management services across all inpatient units
• Manage care coordination for ED holds and overflow units
• Assess, plan, and facilitate patient care to support appropriate utilization of resources
• Collaborate with interdisciplinary teams to coordinate discharge planning and transitions of care

Required Qualifications

• Active Registered Nurse (RN) license required
• Minimum of 5 years of experience as a Registered Nurse
• Minimum of 3 years of case management experience
• In-hospital (inpatient) nursing experience required

Preferred Experience / Skills

• Experience working across multiple inpatient units or service lines
• Familiarity with utilization review and discharge planning processes
• Case Management certification (e.g., CCM or ACM) is a plus
