Contract | 7 Weeks | Mid Shift (3–11) | Every Saturday, Sunday, and Monday | Additional hours may be available for pickup

Key Responsibilities

• Provide direct patient care in a long-term care / skilled nursing facility setting
• Administer medications and treatments in accordance with physician orders and facility protocols
• Monitor and document patient conditions, reporting changes to the appropriate clinical staff
• Collaborate with interdisciplinary care teams to support resident health and safety
• Wear a facility-issued or agency photo badge and present a valid state or federal issued photo ID at all times while on assignment

Required Qualifications

• Active LPN or LVN license in the state of New Hampshire
• Must be local to the area — this position is open to local candidates only
• COVID-19 vaccination required, or a documented religious and/or medical exemption from your employer; additional doses required where mandated by law
• Valid state or federal issued photo identification required at time of assignment

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility environment
• Familiarity with mid-shift (3–11) workflows and staffing expectations
• Strong documentation and communication skills in a clinical setting
