Clinical Care Registered Nurse

Schedule / Job Type

Contract | Full-Time | Days | Monday–Friday, 8:00 AM – 4:30 PM | Onsite

Key Responsibilities

• Triage emergent situations independently, applying advanced nursing knowledge and skills to manage complex clinical scenarios
• Monitor acute and chronic medical conditions in coordination with Primary Care Providers (PCPs)
• Utilize care plans independently to provide both clinic-based and home-based direct nursing care
• Administer treatments, procedures, and medications as prescribed to participants
• Conduct regular home visits to perform wound care, lab draws, nurse assessments, and other nursing tasks as ordered by the PCP
• Collaborate with licensed and non-licensed personnel, staff, participants, caregivers, and external organizations as needed

Required Qualifications

• Associate’s degree in Nursing required; RN Diploma or degree from an accredited School of Nursing or University
• Active Registered Nurse (RN) license in the state of Michigan
• Minimum of one (1) year of experience working with frail, elderly, or long-term care populations
• Current CPR certification (AHA) or willingness and ability to obtain upon hire
• Must possess a valid Michigan driver’s license, reliable transportation, and current auto insurance
• Must be fully vaccinated, or willing to be vaccinated, against Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B
• Must be 18 years of age or older

Preferred Experience / Skills

• Bachelor of Science in Nursing (BSN) preferred
• Minimum of one (1) year of home care experience
• Minimum of one (1) year of wound care experience
