Contract | Days (5×8 Hours, 8:00–17:00) | Tuesday–Saturday, Wednesday–Sunday, or Monday–Friday | Onsite / Field

Key Responsibilities

• Provide physical therapy services to patients in a home health setting across the Central Valley/Stockton/Modesto territory
• Complete patient visits with timely clinical documentation in accordance with program requirements
• Perform and document OASIS assessments as required
• Travel between patient homes using personal transportation; mileage reimbursed at IRS standard rate
• Participate in on-call rotation as needed
• Complete required orientation modules and maintain compliance with all documentation deadlines

Required Qualifications

• Active California Physical Therapist license required (pending CA license accepted if application has been submitted prior to submission)
• BLS certification through the American Heart Association (AHA) required
• OASIS experience required
• Active driver’s license and personal transportation required
• Completion of Home Health Checklist is mandatory
• Local candidates are welcome and will be considered at travel rates

Preferred Experience / Skills

• Minimum of 1 year of home health experience preferred; candidates without home health experience will be considered
• California license in hand at time of submission preferred
