Contract | Days | Full-Time | Onsite | Monday–Friday (flexible shifted schedules available, e.g., Tuesday–Saturday or Sunday–Thursday); expected 40 hours per week

Key Responsibilities

• Provide physical therapy services across both a skilled nursing facility (SNF) and home health (HH) settings, with approximately 50% of time in each setting on a week-to-week basis
• Conduct evaluations and deliver treatment sessions in compliance with SNF productivity expectations (82%) and home health productivity expectations (95%, inclusive of drive time and documentation)
• Supervise up to two therapy assistants as needed
• Document patient care using Net Health and HCHB software platforms
• Travel between patient locations within the home health setting; mileage and drive time between locations are compensated
• Occasional travel to assist in nearby service areas may be requested to support staffing needs

Required Qualifications

• Active Physical Therapist (PT) license in the state of Nebraska or eligibility to obtain licensure
• Ability to work across both SNF and home health care settings
• Comfort with productivity-based performance metrics in both SNF and home health environments
• Valid driver’s license and reliable transportation for home health visits
• Must live at least 80 miles from the facility to be eligible for standard contract rates

Preferred Experience / Skills

• Prior experience in a skilled nursing facility or home health setting
• Familiarity with Net Health and/or HCHB documentation platforms
• Experience supervising physical therapy assistants (PTAs)
• New graduates are welcome — onsite therapy supervision and clinical support are available
