Contract | Days | Field-Based Position | Schedule options may include Tuesday–Saturday, Wednesday–Sunday, or Monday–Friday (5×8-hour shifts)

Key Responsibilities

• Provide occupational therapy services to patients in a home health setting across the Central Valley/Stockton/Modesto territory
• Complete patient visits with timely and accurate clinical documentation
• Conduct and complete OASIS assessments as required
• Manage an independent caseload within an assigned geographic territory using personal transportation
• Participate in on-call rotation as needed
• Complete required orientation modules during onboarding

Required Qualifications

• Active California OT license required at time of submission (pending CA license accepted if application has been submitted)
• BLS certification through the American Heart Association (AHA) required
• OASIS experience required
• Active, valid driver’s license and personal transportation required
• Minimum of 1 year of occupational therapy experience
• Timely communication and documentation are mandatory
• Local candidates are welcome and will be considered at travel rates

Preferred Experience / Skills

• Home health experience strongly preferred; candidates without home health background will be considered
• OASIS experience preferred in addition to being required at a baseline level
• Familiarity with IRS standard mileage reimbursement practices
