Schedule / Job Type

Contract | Days | Preference for Sunday–Thursday or Tuesday–Saturday schedule

Key Responsibilities

• Evaluate and treat skilled nursing facility patients to improve mobility, strength, and overall functional independence
• Develop and implement individualized treatment plans tailored to each patient’s goals
• Collaborate with an interdisciplinary care team to coordinate patient care
• Complete documentation in Net Health in accordance with facility standards
• May be required to float to a sister facility within walking distance

Required Qualifications

• Active California Physical Therapist (PT) License
• Strong documentation and time-management skills
• Flexible and collaborative mindset

Preferred Experience / Skills

• SNF experience preferred; mentorship may be available for candidates with 0–1 years of experience
• Familiarity with Net Health documentation platform
